  • W9 with business or personal name and EIN or Social listed
  • COI (Certificate of Insurance) with 1mil Ag for residential and 2mil Ag for commercial (Elite Companies must be listed as an additional insured, if you need assistance with this our office can help by calling 817-846-6924)
  • Completed Non-Compete and Non-Disclosure
  • Trades Form Listed
  • Customer References
